Passport services are available by appointment only at Main Library and Sylvania Branch. Call 419.259.5201 to schedule an appointment at Main. Call 419.882.2089 to schedule an appointment at Sylvania.
We are a full service Passport Acceptance Agency. We can assist with forms, take your photo, and file the paperwork with the Department of State. Use our checklist below to ensure you have everything you need for your appointment.
Monday – Thursday 10 a.m – 6:30 p.m. Friday and Saturday 10 a.m. - 5 p.m.

First-Time U.S. Passport Book
Adults (age 16+): $110 (Passport fee, payable to Department of State) + $35 (Execution fee, payable to Toledo Library) = $145 Total
Minors (age 0-15): $80 (Passport fee, payable to Department of State) + $35 (Execution fee, payable to Toledo Library) = $115 Total
First-Time U.S. Passport Card
Adults (age 16+): $30 (Passport fee, payable to Department of State) + $35 (Execution fee, payable to Toledo Library) = $65 Total
Minors (age 0-15): $15 (Passport fee, payable to Department of State) + $35 (Execution fee, payable to Toledo Library) = $50 Total

File Search Fee: $150
Expedite Processing: $60
1-2 Day Delivery Service Fee (Passport Book only) = $17.56
Library passport photo: $5.00
Payments made to the Department of State must be made by check or money order.
Payments made to the Library can be made by check, money order or credit card.
